To achieve this mission, the Health Promotion team applies the Center for Disease Control’s (CDC) 10 essential services
and is divided into 4 main components:
Imagine a healthy Bernalillo County, where everyone has the ability to meet his or her full potential. This vision is shared by many within the community. The Bernalillo County Community Health Council serves as a resource and convener for those organizations, institutions and individuals with a passion for improving the health of all Bernalillo County residents. By setting priorities and bringing people together to work toward a shared plan of action, the Health Council provides support to those responsible for creating the conditions needed for health in the neighborhoods where we live, work, learn and play.
Education = Health
Education and health are strongly connected. For this reason, OHSS is piloting a project that attempts to increase basic literacy and help people, street by street and neighborhood by neighborhood, graduate high school or get their GED. We are helping one family at a time, while also helping neighbors in order to support literacy and education in a focused and concentrated way. This will help individuals, families, and neighborhoods get the tools they need to succeed.
Chronic disease prevention: CINCH
Collective Impact for Neighborhood and County Health (CINCH) aims to reduce chronic diseases such as cancer, diabetes, and heart disease in Bernalillo County by supporting policies and neighborhood conditions that promote health. The goal of CINCH is to “make the healthy choice the easy choice.” CINCH is a part of the Community Transformation Grant program administered by the Centers for Disease Control and Prevention.
CINCH staff work with community members and partners throughout Bernalillo County to implement policies that give all residents access to opportunities for healthy eating, active living, healthy and safe physical environments, clinical preventive services, and tobacco-free lives.
