Please note that the fill-in-the-blank “Examples” on the court’s website show only the Application. In addition to the Application, other forms must be submitted to the Probate Court before a personal representative can be appointed. The link to the NM Supreme Court forms takes you to the Supreme Court Law Library, Probate Forms. All forms necessary to begin and complete a probate case, as well as detailed instructions about how to use the forms, are included there. Probate Court staff is not allowed to tell you which forms to submit. If you do not understand which forms to submit, please review the Instructions on the Supreme Court Law Library, Probate Forms web page or click on the court’s link for the “Duties of the Personal Representative” brochure for additional information.
